So you have solved all those physical and chemical problems. How do you want to design from scratch anything with millions of atoms or more? Humans aren´t smart enough to do that, or it will be way too expensive. And won´t the blueprints for any such device be huge computer files in the range of Gigabytes or more?
Short answer
Few systems are designed "from scratch." Most use existing designs as components. Take a skyscraper--it uses off the shelf designs for major structural components (I-beams), lighting (preassembled lamps), elevators (standard OTIS model), etc. etc. The nano-designer will take proven components and combine them to create the next level of complexity.
